4/13/2024 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Laylah was appropriately dressed upon QMHP-C’s arrival. Laylah was in the living room watching tv. Laylah appeared to be in a pleasant mood, as evidenced by her calm and subtle facial expressions and discussions. QMHP-C facilitated one on one session with Laylah to review being kind to others and not letting her emotions get the best of her. QMHP-C administered the session to enhance Laylah’s ability to find a hobby or learn a new skill. Intervention: QMHP-C engaged in conversation with Laylah about upcoming 3-day weekend plans. QMHP-C asked Laylah if she practiced her calming strategies over the weekend. QMHP-C processed and reiterated with Laylah about the rules in the home. QMHP-C assisted Laylah with understanding the expectations that have been established in the home and community. QMHP-C went over social skills activities with Laylah to help her understand the importance of following rules. QMHP-C activity helped Laylah state, demonstrate, or identify how to communicate her feelings. QMHP-C processed with Laylah the thinking questions to each social skill activity. QMHP-C will help Laylah develop healthy self-esteem by reducing level of anxiety and emotional reactions related to home or school and other independent functioning. QMHP-C praised Laylah for being on task and following directions. QMHP-C processed Laylah thoughts and feelings. QMHP-C engaged Laylah in conversation regarding her recent behaviors, progressions, and setbacks. QMHP-C encouraged Laylah to apply these social skills to when she goes back to school. QMHP-C informed Laylah that we need friendships and positive relationships so that we learn how to give, accept, and receive constructive criticism and support from others. QMHP-C told Laylah that our interpersonal relationships shape our minds and uplifts our spirits when we are sad or lonely. QMHP-C educated Laylah that having a strong support system can uplift our mood, reduce stress and anxiety, improve our self-esteem, and hold us accountable for our actions. QMHP-C encouraged Laylah to identify how to be kind to everyone she comes in contact with. QMHP-C praised Laylah for the feedback during the session and showing more effort to contribute to discussion. During the first part of the session, Laylah’s mother sat and listened as the treatment plan goals and the plan of actions were read off to her. Laylah’s parent agreed to all the terms and conditions and made no objections to the treatment plan as she signed it and shared her own questions and concerns for her daughter. Mom thanked QMHP-C for her patience and service with wanting to help her daughter do better in all aspects of her life especially at home. Response: Laylah also followed along and answered questions on the topic. Laylah demonstrated a nonchalant attitude but displayed good eye contact and positive facial expressions throughout the entire session. Laylah displayed active listening skills during the session, as she nodded her head and responded back to feedback to assure the counselor knew she was engaged in the discussion. Laylah does not seem to like being on punishment or losing privileges and money, as evidenced by her poking her lip out and crossing her arms at minor irritants. Laylah communicated that she has been implementing acquired skills to deal with her stress and anxiety, especially crying. Laylah promised to stay focused with meeting her goals and objectives at home and at school on a daily basis. Laylah participated and cooperated well with the social skills activities. Laylah was able to answer the thinking questions with assistance from QMHP-C. Laylah asked appropriate questions concerning the social skills activities. Laylah implemented appropriate social interaction skills. Laylah displayed good study and organizational skills. Laylah displayed an understanding of ADHD and its symptoms and causes. Laylah was observed and reported eliminating personal conflicts at home. Laylah utilized conflict management skills. Laylah exhibited techniques for appropriate self-expression and active listening. Laylah communicated positive self-talk and affirmations. Laylah was able to recognize personal strengths and abilities to perform better at home. QMHP-C will continue to assist Laylah in respecting the home, interacting and engaging appropriately and continue to review her behavioral plan and goals daily. QMHP-C will implement different forms of coping to manage stress and emotions in a constructive manner. QMHP-C will also implement rule compliance and increase respect for authority on a daily
